Demi Lovato was hospitalized for a heroin overdose just after noon today in LA, TMZ reports. The troubled singer was transported from a home in the Hollywood Hills after being found unconscious, and is currently receiving treatment. Details of her condition are not known at this time, so we really hope she's OK and makes it through.

The chart topper came clean about her relapse with drugs and alcohol in June in her new song "Sober," but reports claimed that she was working on regaining her sobriety following the confession.

The 25-year-old has a long history with mental health issues and substance abuse. She's been in and out of rehabs for cocaine, eating disorders, self-harm, and bipolar disorder, but she managed to stay clean for over six years up until last month.

Celebrities are already taking to social media to react to the sad news. "I love @DDLovato so much. It breaks my heart that she is going through this. She is a light in this world, and I am sending my love to her and her family," Ellen Degeneres tweeted. "i love u @ddlovato," Ariana Grande added. "Poor beautiful spirit @ddlovato I hope she’s ok, and that she makes a full recovery soon," Lily Allen wrote.

Just 22 hours ago, she posted this photo of her new blonde hairdo on Instagram. We guess things aren't always what they seem!

UPDATE: People reports that Demi is now in "stable" condition after being revived with Narcan.
