Realistic, poor hygiene is often a quick sign of many underlying issues, especially mental illness and drug abuse. It seems obvious that she will do anything to avoid the stigma of either mental health facilities, or rehabs. She may not be doing illicit drugs right now, especially with being under constant surveillance she's under from her own people and the paps, but the drugs she is prescribed can be very powerful and easily misused or combined with other things. As far as mental illness goes, only idiots and blind fans think that there is some sort of complete cure for it. People like to blame everything on anyone else that's been around her and while there is truth there to some degree, even her own father said that she created the problems she has and that the other people just preyed on those weaknesses. That was a massive thing for him to say that and if anyone would know the score, it would be him.
