WASHINGTON - The hottest topic in cocaine addiction is another drug -- a medicine already sold to wake up narcoleptics.

Hundreds of cocaine users are testing whether that legal pill, called modafinil, could help them kick the addiction, and there's early evidence that it may.

In addition to blunting cocaine's notorious cravings, modafinil might also counter the damage that cocaine wreaks on users' brain circuits -- damage that, in turn, fuels the cycle of addiction.

The prospect of that double-whammy has the National Institutes of Health spending $10.8 million researching modafinil as a potential cocaine treatment.

Results from the first of three key clinical trials could arrive by year's end.

Scientists are cautious.

In a hunt spanning two decades, no one has found a medication to help treat cocaine addiction, and there's no guarantee that modafinil will pan out.

But for Dr. Nora Volkow, director of the NIH's National Institute on Drug Abuse, the narcolepsy medicine tops the list of promising potential therapies.

It may help restore proper function of a crucial brain chemical, dopamine, that addiction hijacks.

And in describing why he's hopeful, one leading researcher recounts the man who accused his drug dealer of selling bad coke before realizing modafinil had kept him from getting high -- and several other modafinil testers who told of flushing cocaine down the toilet.

''I've been treating cocaine-addicted patients for something like 25 years, more, and I've never heard of anybody throwing away cocaine,'' said Dr. Charles Dackis, of the University of Pennsylvania.

He led a pilot study that suggested modafinil more than doubled cocaine addicts' chances of going cocaine-free for a period of at least three weeks.

That study enrolled just 62 people, but the results were significant enough for the NIH to fund new research -- at Pennsylvania, the University of Texas in Houston, Boston University and other sites -- enrolling about 650 cocaine users to see if modafinil really does work.

The main side effect so far: insomnia, not surprising as modafinil is sold today to help narcolepsy patients fend off that neurological disease's sudden sleep attacks.

Addiction specialists gave it a look because even though modafinil isn't a classic stimulant, it triggered something in the brain to also improve patients' mood, energy levels and ability to concentrate -- effects that might counter cocaine withdrawal. Then came the surprises:

* Cocaine intensely stimulates the brain's pleasure centers, producing not just a ''buzz'' or a ''rush,'' but outright euphoria. In a small safety study to ensure that modafinil didn't make cocaine worse, some users found the pill blocked that high.
* Modafinil seems to increase activity in the prefrontal cortex, the brain's decision-making command center and the spot that allows reasoning to override impulse or emotion. Cocaine reduces activity in that key brain region, making it even harder for would-be quitters to ignore cravings.
* Modafinil also increases the ability to think strategically, a means of weighing variables and risks to make decisions, says Frank Vocci, NIDA's pharmacotherapy chief.

Cocaine is highly addictive: About 16 percent of people who try it become hooked.

In the latest data, from 2003, the government estimated that more than 1.5 million Americans were dependent on or abusing cocaine, and more reported recently experimenting with it.

Several studies found Sparlon effective for AD/HD in approximately 50% of children, adolescents, and adults.

Since Sparlon works on different neurotransmitters than traditional stimulants used to treat AD/HD, it is sometimes effective for people who don't respond to other stimulants. It is also not a controlled drug and refills are permitted.

Sparlon was awaiting approval from the FDA. Unfortunately, the FDA advisory panel recommended "non-approval" because of concerns over the possibility of a rare but serious rash, and recommended the drug company conduct a 3,000-patient clinical trial to rule out its occurrence. Cephalon decided to not proceed with additional studies.

This decision does not mean that a physician cannot prescribe Provigil for AD/HD, but it is an expensive drug that might not be covered by someone's insurance if not prescribed for approved conditions, such as narcolepsy.

Celebrity News: Kevin Federline Puffing Pot At Work?

Naughty Kevin Federline has reportedly been smoking marijuana on the set of his new TV show.

The former backing dancer - who is currently locked in a bitter custody dispute with ex-wife Britney Spears – is said to have stunned cast and crew on the North Carolina set of hit TV show One Tree Hill by openly puffing on joints.

A source snitches on K-Fed to the New York Daily News: "He is smoking marijuana all round the set, like it is just part of his lifestyle.

"The smell is so potent it's obvious that he is not just smoking regular cigarettes.

"He doesn't seem to see a need to hide it at all."

Earlier this month, Kevin won custody of his two sons with ex-wife after Britney failed to attend a court-ordered drugs test.

The next hearing is set for October 26.

Showbiz Tonight did a piece about Britney and Kevin and they raved about Kevin and spoke about the TV show he is on and that he was polite and hard working on the set. They also said he seems to genuinely have changed and that he, at the moment, is the better parent.

They said Britney is a drama queen and that she she won't get the public's sympathy until she shows real strides at shaping up and listening to authority.

Someone who was on the set of One Tree Hill (or whatever the hell it is, never heard of it) said Kevin was quiet and did what he was told and caused no issues. No mention of this "open pot smoking" that Britabloids made up.

I saw that about K-Fed's pot smoking on the set too. The Arizona Republic reported that more a week ago and I relayed the info here but nobody was interested. I also read a news report about how K-Fed made the bar and nightclub rounds when he was in Wilmington and that he was drinking and partying "like a rock star." A nightclub cocktail waitress told People.com that K-Fed brought in his entourage and partied until after 2:00am and that he was drinking Jack Daniels & Coke and tequila shots. I don't know why the major celebrity news sources ignore stuff like this about K-Fed while this custody decision is being argued but it is obvious they do. Actually X17 did report it but most of the blogs were spewing hate towards x17 because they posted a report that was against Kevin.The hate was awful, this site was blasted because some of the bloggers didn not want to believe it (their choice) & only wanted Brit to look good. I was on Provigil for one year after I had my daughter. It Depends on the persons needs and the chemical balance in the person's brain. I just so happen to have ADHD and at the time my internal clock had it's days and nights confused. This medication if taken correctly can help fix this problem. I was staying up all night and sleeping most of the day. The medication gave me enough energy to where I was so wired during the day, by the time it got to 10:00PM I would crash. So yes if you have ADHD it can be a curse, but if you want to have a normal sleep pattern again it can be a blessing. Hope this helps people and they stop making stuff up. Remember don't judge someone unless you have been in there shoes. It's God's job to judge not ours.
